Prepare an NDIS referral by gathering the information you have. You do not need every detail before speaking with us.

Before you get started
You do not need to have everything worked out before contacting us. However, a few key details can help us understand what support may be needed and whether The Diverse Collective may be a suitable fit.
What information is helpful?
When making a referral, it can be helpful to have:
- The participant’s name and contact details
- Where they live and the areas where support is required
- The type of support they are looking for
- Their current NDIS plan or relevant funding information, where available
- Their preferred days, times or frequency of support
- Their goals and what they would like support to help them achieve
- Any communication or accessibility requirements
- Information about important support needs, risks or circumstances we should understand
- Contact details for their nominee, family member or another relevant person, where appropriate
Even so, you do not need to have all of this information before getting in touch. We can explain the next steps and discuss what else may be helpful.
Tell us what good support looks like
A referral is about more than funding and service hours. For example, it helps to share what is important to the person and what they want from their supports.
This may include interests, routines, preferences, communication style, goals and what has or has not worked well in the past. As a result, we can consider whether we have the right people, experience and current capacity to provide support that is a good fit.
Complex support needs are welcome
The Diverse Collective has particular experience supporting people with psychosocial, neurological, intellectual and complex disabilities, including people with complex behaviours and support needs.
Therefore, you do not need to simplify someone’s circumstances when making a referral. Relevant information helps us make an informed decision about whether we can safely and appropriately meet the person’s needs.
What happens after you make a referral?
First, our team reviews the information you provide. Then, we may contact you to learn more about the person, their preferences, location, requested supports and our current capacity.
Where appropriate, the next step may include a conversation or meet and greet. This gives the participant an opportunity to learn more about us too. Making a referral does not commit anyone to receiving services from The Diverse Collective.
Who can make a referral?
A participant can make their own referral. Alternatively, with appropriate authority or consent, a referral can come from someone supporting them, such as a:
- Family member or carer
- Plan nominee
- Hospital or health professional
- Allied health professional
- Government or community service
- Advocate or other support person
